An unannounced radiological controls inspection was conducted on August 2-6,1993, by Mr.

R. L. Nimitz at the Millstone Nuclear Power Station Units 1, 2, and 3. The findings of the

inspection were discussed with Mr. R. Factora and others at the conclusion of the inspection on

August 6,1993.

Areas reviewed during the inspection were important to health and safety and are discussed m

!

the enclosed inspection report. The areas reviewed included program changes, organization and

i

staffing, training and qualifications, audits and assessments, efforts to maintain radiation

exposures as low as is reasonably achievable (AIARA), external and internal exposure controls,

instrument calibrations, control room ventilation system testing, radioactive material and

contamination controls, process monitor testing, and station housekeeping. The inspection

principally focused on the planning and preparation for, and the adequacy and implementation

of, radiological controls for the refueling outage at Millstone Unit 3.

The inspector also

reviewed activities at Units 1 and 2 including the entry of personnel into Unit 2 containment,

with the reactor at power, to perform leak repair activities on a letdown valve.

The inspection revealed that, overall, very good radiological controls were implemented for the

Unit 3 outage and the work activities reviewed at Units 1 and 2. Areas that were particularly

noteworthy included planning and coordination of work activities for the outage, the general

material condition of the station, and control of radioactive and contaminated material. The

radiological controls provided for the repair of the Unit 2 letdown valve were considered good.

Your efforts to maintain the conditions inside the Unit 3 reactor containment to allow personnel

entry with street clothing was commendable. Also, station efforts in the ALARA area continued

to be very good.

One unresolved item was identified. The item involved the training and qualification of

personnel performing instrument calibrations or using instruments. The inspector identified

three individuals for whom complete records of training and qualification to perform their

assigned tasks were not available. Your staff took immediate actions to evaluate this matter and

,

assure personnel had or would be pmvided appropriate training. Discussions with your staff

i

indicated on-the-job training had apparently been provided for the individuals but records were

not available. We will review this matter during a future inspection.

No response to this letter is required. Your cooperation with us is appreciated.
