20555 Dear Mr. MacDonald.
On August 12, 1991. I wrote you a letter, asking that our Quality Assurance Program. No. 0294. be terminated.
At that t.ime we were Ultrason2e Specialist Inc. and the president and owner, did not wish to continue with the QA procram because of the increase in the annual fee to $1800.00 We have been purchased by Longview Inspection Inc. and our name has been changed to Longview Inspection.
I am requesting that we be issued a Quality Assurance Program that would allow us to be an author 2:ed user of Certificate of Compliance Nos. 6717 9028, 9032. 9033. 9035. 9056. 9126. 9127.
9129. and 9135.
I am sure you will require additional information and the fee.
