Quality Assurance Program Approval No. 0656 2.0 ORGANIZATION Provide an organization chart identifying each organizational element that functions under the Quality Assurance (QA) program.
3.0 QUALITY ASSURANCE PROGRAM State that the management of Professional Welding Associates, Inc.,
establishes and implements this QA Program. Training, prior to engagement,

for all QA functions is required according to written procedures. QA Program revisions wi'l be made according to written procedures and with management approval.
11e QA Program will ensure that all defined QA procedures, engineering irocedures, and specific provisions of the packaging design approval are satisfied.
The QA program will emphasize control of the characteristics of the package which are critical to safety.
4.0 DOCUMENT CONTROL State that all document changes will be performed according to written procedures approved by management.
State that the Radiation Safety Officer shall ensure that all QA functions are conducted in accordance with the latest applicable changes to these documents.
7.0 QUALITY ASSURANCE RECORDS State that descriptions of equipment, written procedures, and records of personnel training and qualifications will be retained for three years beyond the date activity, for which this QA Program was developed, ends.
8.0 AUDITS State that established schedules of audits of the QA program will be performed using written check lists.
