SUBJECT: NRC INSPECTION REPORT NO. 50-213/93-12
This letter transmits the report of the resident safety inspection conducted by
Messrs. W. Raymond and P. Habighorst of this office during the period June 20,1993 to
July 24,1993, at the Haddam Neck Plant, Haddam Neck, Connecticut. The report covers
activities authorized by NRC License No. DPR-61. This inspection was focused on issues
important to public health and safety. Preliminary findings were discussed with Mr. J. Stetz
of your staff at the conclusion of the inspection.
During this report period, we found Haddam Neck was operated in a safe manner as the 1993
refueling outage was completed. Operator responses to a reactor trip and to loss of power
events were very good. Plant personnel also responded well to significant challenges and

emergent issues during the period. In particular, the loss of power events and several
equipment performance issues were thoroughly evaluated, with good root cause
determinations and appropriate corrective actions. Where no definitive root causes were
,
identified (e.g., the loss of power to motor control center No. 5 on June 27), good proximate
,
causes were identified and addressed. Plant personnel performed well to complete the
outage, conduct a plant startup, and resume power operations. One exception to otherwise
good control of plant system alignmen's was the discovery by plant personnel that main steam
line flow instruments were isolated in Operational Mode 3. In recognition of the prompt and
comprehensive corrective actions, enforcement discretion was exercised for this event.

The enclosed inspection report also describes several issues which are of concern to the NRC,

including a reactor trip, two loss of power events, two challenges to the plant overpressure
protection system and an emergency classification that was miscommunicated as an Alert. It
is noted that some of these events resulted from expanded testing that identified longstanding
equipment or design problems. However, these events raise concerns regarding the adequacy
l
of procedure reviews and the self-checking by plant personnel of their actions during the
performance of routine duties. These matters will be the subject of further assessment by the
NRC.
