' Weakness (498/9317-01; 499/9317-01) , , Failure to recognize General Emergency conditions resulting from , overall plant conditions. Response I The root cause of this weakness is a lack of emphasis during emergency classification training to evaluate overall plant conditions in
determining emergency classification. Emphasis had been placed on ! identifying a particular Emergency Action Level to properly classify an f emergency. L
Corrective Actions
. 1. Monthly emergency classification seminars will be conducted for , ' people involved in the emergency classification process, with , emphasis on the need to evaluate overall plant conditions as a part of the emergency classification process. The seminars will include a discussion of the bases used in the assessment followed by a 1 tabletop drill. The first seminar to cover this topic will be ! conducted in August. The series will be complete in October. t { ' 2. Training material for initial training will'be revised to emphasize overall plant conditions when making emergency classifications in addition to events based on Emergency Action Levels. This change
in emphasis will begin in December, 1993. j Weakness (498/9317-02; 499/9317-02) Performance was lacking in providing technical assessment, diagnosis, { , and mitigative activities in the Technical Support Center.

Support Center, and the Control Room. The seminars will include
performance based tabletop exercises to evaluate the effectiveness . of this training. This series of seminars will be completed by l December 1, 1993. 2. During initial training of personnel nominated to the Emergency
Response Organization TSC technical, operations, or radiological ! staffs, position based training will be conducted to ensure understanding of their role in assessing, diagnosing, and j mitigating plant casualties. This enhanced training will begin for
personnel assigned after December, 1993. l ! 3. Drills and exercises for the TSC will be conducted. Special ' i emphasis will be given to evaluating the ability of the TSC to l provide technical assessment, diagnosis, and mitigative activities. ! ' Comprehensive feedback will be provided to the TSC technical staffs on their group and individual performance. Weaknesses noted will , be tracked and trended to ensure correction. , t ! Weakness (495/9317-03; 499/9317-03) Sufficient administrative staffing was not present in the Technical Support Center. In addition, steps were not taken to obtain additional staffing or to reassign the missing staff's responsibilities. } Response The root cause of this weakness was failure to recognize the potential i impact of missing staff members and take compensatory actions to
, mitigate this deficiency. Corrective Actions OSC 1. Training will be conducted for EOF Directors, TSC Managers, Coordinators, and Control Room Shift Supervisors that emphasizes the importance of all staff members and compensatory actions that shall be taken in the event a staff member does not report. This s training will be completed by December 1, 1993. ' 2. During initial training of Emergency Response Organization personnel nominated to fill facility manager positions, a topic will be included that describes facility positions and compensatory actions that could be taken should a staff member not report to the facility. Response . The root causes of this weakness were the ambulance was parked outside l the Protected Area and the person assigned to drive the ambulance had > no formal training in its operation. Contributing to the delay were deficiencies in the realism of the scenario resulting in a lack of , urgency to address the simulated injury. , Corrective Actions 1. An ambulance will be permanently parked inside the Protected Area. i The on-shift General Maintenance Supervisor will be trained on the operation of the ambulance and will be responsible to provide a trained driver from the shift staff. These actions will be , completed by August 12, 1993. 2. In future drills and exercises, more emphasis will be placed on describing the victim's condition and the surrounding environment to ensure that care attendants have sufficient information to develop the appropriate responses. I Weakness (498/9317-05; 499/9317-05) Not all offsite notification messages were complete or accurate. l Response The root cause of this weakness was a failure to clearly describe expectations of accuracy and completeness in the message preparation to those preparing and approving the messages. Corrective Actions ' Training will be conducted for personnel responsible for offsite 1. agency' notification messages preparation and approvalto emphasize i the need to complete all required entries neatly'and accurately. This training will be completed by October 29, 1993. Training drills will initially be conducted quarterly for personnel
