Task Order No. 4 entitled, "All Employee Training Survey" Under Contract No. NRC-38-92-281 This confirms verbal authorization provided to you on April 09, 1993, to commence work under Task Order No. 4, with a temporary ceiling of $5,000.00.
In accordance with the task order procedures of the subject contract, this letter definitizes Task Order No. 4.
This effort shall be performed in accordance with the enclosed Statement of Work.
Task Order No. 4 shall be in effect from April 09, 1993 through October 30, 1993 with a cost ceiling of $25,225.00.
The amount of $23,311.00 represents the total estimated reimbursable costs and the amount of
$1,914.00 represents the fixed fee.
The obligated amount of this task order is $25,225.00.

STATEMENT OF WORK BACKGROUND:
j The Organizational Development and Training Component in the Nuclear Regulatory Commission's Office of Personnel (OP) is in the process of developing an all-employee survey to be administered in the Spring of 1993 to solicit opinions on work-related topics for use in enhancing training and performance management.
CONTRACT OBJECTIVES:
OP requires contractor assistance in designing, developing, and administering the survey, analyzing the results of the survey, and comparing.
NRC's training with the training in other Government agencies based on the results of the NRC survey and the Office of Personnel Management's (0PM'S)
Survey of Federal Employees.
The contractor will also be required to provide draft and final reports containing the results of the NRC survey and how they compare with results obtained from the OPM-administered survey.
WORK TO BE PERFORMED AND EXPECTED RESULTS:
Task 1:
The Contractor shall design and develop a survey form.
Based on a draft questionnaire provided by the NRC at time of award of this task order, the Contractor will mark up and compose a survey form, suitable for optical scanning. The survey shall consists of approximately 100 close-ended questions using Likert-like scales and up to five open-ended questions. -The Contractor shall submit.
the survey form to the NRC Project Officer for review and comment within two weeks from the receipt of the draft questionnaire.
NRC will provide any necessary changes to the Contractor within two weeks from the receipt date of the survey form.
The Contractor shall then incorporate these changes in the final survey-and submit to the NRC Project Officer for approval within three days from receipt of the NRC changes.
The Contractor shall print all survey forms and provide labels for the return envelopes to administer the survey to all NRC Headquarters and Regional Employees.
The return envelopes shall be addressed to " Organizational Development and Training, Office of Personnel, Mail Stop W-316", and the survey form shall accompany the return envelopes under a separate cover, later to be inserted inside the envelopes by NRC, due to the scanner Responses to open ended questions shall be recorded and analyzed by the contractor using the least expensive technique, i.e. print out all responses i

Task 1 unable to scan stapled or folded survey forms. Approximately Cont'd 3,500 copies of the survey form will be required. All materials (e.g., printed survey forms and return envelopes) will be shipped to the NRC within 6 weeks of NRC's approval of the survey form.

Task 2:
The NRC will provide the completed surveys to the Contractor four i
weeks after the survey has been administered to all employees.
The Contractor shall be responsible for data preparation, statistical analyses, and cross tabulations (further explanation is provided in Task 3 below).
Task 3:
The Contractor shall analyze data obtained from the completed surveys and deliver a draft report, I hard copy and 1 copy of a diskette in Wordperfect 5.1, to the NRC Project Officer eight weeks after receiving the surveys.
Subtasks to be performed by the contractor include:
(1) running of tabulations, including :

o cross-classifications, (2) comparing results of NRC's survey with those obtained in the OPM governmentwide survey, and (3) developing a draft report summarizing statistical findings and how the results of the NRC survey compare with findings from the OPM governmentwide survey

The Contractor shall prepare a final report incorporating NRC's comments on the draft report within four weeks from receipt of

the required changes. The draft report shall be submitted in 1 hard copy and I copy of a diskette in Wordperfect 5.1.
