4, ENTITLED "LER REVIEW AND DATA INCORPORATION INTO HUMAN FACTORS INFORMATION SYSTEM" UNDER CONTRACT NO. NRC-03-93-036 In accordance with Section G.5(c) of the subject contract, entitled

" Task Order Award," this letter definitizes the subject task order.

This effort shall be performed in accordance with the enclosed Statement of Work, and the Contractor's proposal dated April 1,

1993.

Task Order No. 4 shall ba in effect from April 30, 1993 through September 16, 1993, with a cost ceiling of $36,726.00.

The amount of $33,796.00 represents the total estimated reimbursable costs, the amount of $2,788.00 represents the fixed fee, and the amount of

$142.00 represents the facility's capital cost of money.

Funding for this task order has been obligated on the basic contract under Job Code No. J-2015-3.

The following individual is considered to be essential to the successful performance of the work hereunder:

2 BACKGROUND During 1990 and 1991, the NRC Human Factors Assessment Branch began the development and implementation of a database referred to as the Human factors Information System (HFIS). The HFIS database is designed to be used by the Staff to collect and compile human performance data related to nuclear power plants.

The HFIS consists of five modules, one of which provides detailed human factors information extracted from LERs to enable the user to identify patterns related to human performance and underlying contributing factors which may have been implicated in the event.

The HFIS currently contains human performance information derived from LERs from 1990 and 1991.

In order to keep the HFIS data up to date, the LERs for 1992 should be coded and entered into the system.

OBJECTIVE The objective of this task order is to obtain technical assistance to review LERs for the year 1992 using the HFIS Event Analysis Protocol and incorporate information into HFIS. Based on previous experience, it is estimated that approximately 1200 human performance' related LERs were generated in 1992.

I TECHNICAL AND OTHER SPECIAL QUALIFICATIONS REQUIRED One Human Factors specialist with expertise in evalualing human p'erformance in order to identify pertinent performance.information for incorporation into the HFIS database.

Several part time LER reviewers / data entry personnel. These personnel should have some training in human factors issues.

It is the responsiSility of the contractor to assign technical staff, employees, subcontractors, or specialists who have.the required educational background, experience, or combination thereof to meet both the technical and regulatory objective of the work specified in this statement.of work.

The NRC will rely on representations made by the contractor concerning the qualification of the personnel assigned to this task order including assurance that all information contained in the technical and cost proposals, including resumes, is accurate and truthful.

I WORK RE0VIREMENTS AND SCHEDULE The contractor shall provide the qualified personnel to complete the following tasks.
