Dear Ms. Slayer:

In accordance with Section G.4, Task Order Procedures, of the subject contract, this letter definitizes task order number 9 with an established ceiling of $25,959.91. This effort shall be performed in accordance with the enclosed statement of work, the contractor's proposal dated June 3,1993, and the following telephonic changes of June 14, 1993:

Reduce estimated travel costs by $13.62. Reimbursement re'.e for mileage is $0.25 per mile.

Reduce fixed fee from $2,350.86 (10%) to $1,973.41 (8.4%)

The period of performance for task order number 9 is June 15, 1993 through December 15, 1993. The total cost ceiling is $25,959.91, of which the sum of

$23,492.98 rapresents the estimated reimbursable cost, the sum of $133.52 represents facilities capital cost of money, and the sum of $1,973.41 represents the fixed fee.

The amount presently obligated with respect to this task order is $25,959.91.

Notwithstanding the clause FAR 52.232 LIMITATION OF COST,'the contractor is not obligated to continue performance under this task order or otherwise incur costs in excess of the estimated cost, until the Contracting Officer notifies the contractor in writing that the estimated cost has been increased.

In absence of such notification by the Contracting Officer, the Government is not obligated to reimburse the contractor for any costs in excess of the estimated cost specified herein.

4 STATEMENT OF WORK TASK ORDER NO. 9 UNDER CONTRACT NO. NRC-26-91-292 f

1.

PURPOSE The purpose of this Task Order is to present the Internal Dosimetry Course (H-312). This is a five (5) day training course for NRC Inspectors who have a need to understand the concepts of determining dose from internal radioactive sources. The presentation will be based on the manual developed by the contractor under Task Order 6 and should provide the inspectors with the knowledge necessary to inspect licensee activities which could involve intakes of radionuclides. The course will be presented at the NRC Technical Training Center, Chattanooga, Tennessee.

2.

SCOPE OF WORK The contractor shall administer the course and provide instructors to teach the course. The contractor shall utilize the course manual, instructor's guide, and visual aids developed under Task Order 6.

The contractor shall include discussions / workshops dealing with internal dose calculations.

The contractor shall evaluate the students by means of a written examination.

TRAVEL Contractor travel to support the presentation at the Technical Training Center will be necessary.

5.

REPORTING REQUIREMENTS / DELIVERABLES t

The contractor shall:

a.

Submit reports in accordance with Section F of the contract.

b.

Provide experienced instructor (s) to present the course in Chattanooga, TN. The resume (s) of the proposed instructor (s) shall be provided to the Task Order Leader for review and approval as soon as the individual (s) have been identified unless the resumes have been provided, t

previously.

c.

Conduct the course during the week of June 28, 1993 to July 2, 1993.

Utilize the student manual, instructor's guide, visual aids and other materials developed under Task Order 6.

e.

Administer examinations developed under Task Order 6.

The examinations should consist of at least 50 questions, predominantly multiple choice and should be based on the material included in the student manual.

Grade the examination upon completion of the course and transfer the examinations along with the raw scores to the Task Order Leader as specified in g. below.

g.

Submit a course presentation report to the NRC Task Order Leader within thirty (30) days of completion of the course presentation. The report shall contain:

(1) A cover letter report discussing course accomplishments, i

problems, and recommendations for improvement.

(2)' Original graded examinations and examination key, i

Summary report of student evaluations of the course and the original of each student's evaluation.

6.

PERIOD OF PERFORMANCE I

The period of performance of this task order shall be six (6) months from the date of issue.

7.

ESTIMATED LEVEL OF SUPPORT Approximately one and one half (1.5) staff-month. Estimate includes preparation for and presentation of the course.

8.

TECHNICAL SKILLS REQUIRED The contractor shall provide resumes of each person selected to present the course if the resume was not previously provided to the NRC Project Officer. Specific skills required are knowledge and experience in the subject areas being taught and experience in classroom presentation and course evaluation.

9.

GOVERNNENT PROPERTY TO BE PROVIDED TO THE CONTRACTOR The NRC will furnish the contractor with classroom facilities and equipment (e.g., overhead and slide projectors, marker boards, video projection system, etc.) needed to present the course.

GENERAL INFORMATION j

The standard instruction period is eight (8) total hours per day beginning no earlier than 7:30 a.m. and ending no later than 5 p.m. with one (1) hour set aside for lunch.
