During an NRC inspection conducted on July 12-16, 1993, a violation of NRC requirements was identified.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," the violation is listed below i
10 CFR 50 Appendix B, Criterion V, Instructions, Procedures, and Drawings, and the licensee's accepted quality assurance program, Topical Quality Assurance Report (FPLTQAR l-76A) Revision 29, TQR Section 5, requires that activities affecting quality of nuclear safety-related components shall be prescribed by i
documented instructions of a type appropriate to assure important activities i
have been satisfactorily accomplished.
For activities involving safety-related cable pulls, the licensee's plant specification 5610-E-756, Installa-tion, Inspection, and Testing Details for Electrical Equipent and Cable, Revision 8, requires a calculated pull stress limit and pull stress measuring device be used when accomplishing a mechanical cable pull.
Contrary to the above, the work procedure used for overhaul of the 3B charging l
pump and pull of the motor lead cables on May 20, 1992, WA920319860000, was i
inadequate to assure important activities were satisfactorily accomplished. A i
l mechanical cable pull was performed without calculation of a pull stress limit l
or use of a device to measure the induced pull stress on the cable. Work i
l instruction WA920319860000 was inadequate in that it directed the craft to i
pull the 3B charging pump motor lead cable but did.not provide instructions l
conforming to the applicable plant specification nor reference the specifica-l tion.
This is a Severity Level IV violation (Supplement I). This applies to Unit 3.
l Pursuant to the provisions of 10 CFR 2.201, Florida Power & Light is hereby I
required to submit a written statement or explanation to the U.S. Nuclear i
Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 l
with a copy to the Regional Administrator, Region II, and a copy to the NRC i
Resident Inspector at the facility that is the subject of this Notice, within j
30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice.of i
Violation" and should include for each violation:
(1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the i
date when full compliance will be achieved.
