Gentlemen:

SUBJECT:

NRC INSPECTION REPORT N0. 50-327/93-26 AND 50-328/93-26

This refers to the inspection conducted by G. MacDonald of this office on

June 14-23, 1993. The inspection included a review of activities authorized

'

for your Sequoyah facility. At the conclusion of the inspection, the findings

were discussed with those members of your staff identified in the report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

i

activities in progress.

Within the scope of the inspection, violations or deviations were not identi-

fled.

j

An event occurred on June 14, 1993, where an incorrectly installed current

transformer in circuit breaker 1614 tripped during a reactor coolant pump

motor start causing an unplanned challenge to the emergency diesel generators.

We are concerned about this problem because it demonstrated a breakdown in

your control of maintenance in the areas of configuration management and post

maintenance testing.

This event was very similar to an incident which occurred on December 19,

1991. During the December 19, 1991, incident an incorrectly installed current

transformer led to a trip of the main generator.

This indicates that your

corrective actions for the December 19, 1991, incident were inadequate to

prevent recurrence. We request that you respond to this item and provide your

proposed corrective action.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

this letter and its enclosure will be placed in the NRC Public Document Room.

The responses directed by this letter are not subject to the clearance

procedures of the Office of Management and Budget as required by the Paperwork

!

Reduction Act of 1980, Pub. L. No.96-511.
