SUBJECT: NRC INSPECTION REPORT 50-498/93-19; 50-499/93-19

This refers to the reactive inspection conducted by Messrs. L. E. Ellershaw  :

and C. J. Paulk of this office on May 24 through June 14, 1993. This

inspection included a review of activities authorized for your South Texas

Project, Units I and 2, facility. At the conclusion of the inspection, the

findings were discussed with those members of your staff identified in the  !

enclosed report. Additional information was discussed with Mr. M. Coughlin on  ;

June 14, 1993; and Ms. P. Parrish on June 23, 1993; and Mr. M. Cooghlin on

June 29, 1993.

Areas examined during the inspection are identified in the report. These 4

areas included followup on your notification regarding a normally closed main

feedwater isolation bypass valve being discovered partially open, and

notification of a failure to maintain the equipment qualification of a

residual heat removal motor-operated valve. Within these areas, the -

inspection consisted of selective examinations of procedures and '

representative records, interviews with personnel, and observation of

activities in progress. The results of this inspection are documented in the ,

enclosed report. j

Based on the results of this inspection, six unresolved iteras were identified.

These unresolved items involved safety-related classification, solenoid aging,

calibration of remote position indicators, and correction of an identified

design deficiency of main feedwater isolation bypass valves, and T-drain

installation on a residual heat removal motor-operated valve. In additior.,

please be advised that the number and characterization of unresolved items

described in the enclosed inspection report may change as a result of further

NRC review.

A management meeting to discuss these unresolved items will be scheduled with 1

you by separate correspondence. In particular, we request you to address the  !

safety significance regarding the apparent degradation of the main feedwater

isolation bypass, valves. Also, please be prepared to discuss your position

regarding the need to request an amendment to Technical Specifications related ,

to the operability of main feedwater isolation bypass valves. Lastly, we

l request that you address the root causes which allowed these problems to

occur. You will be advised by separate correspondence of the results of our

deliberations on this matter. No response regarding these unresolved items is

required at this time.

l It is our understanding that during the preliminary exit coaducted on May 28,

i 1993, Mr. G. Parkey, Plant Manager, committed to implement Design Change

ECNP 89-J-0165 prior to unit restart, which would result in Unit 2 main

feedwater isolation bypass valve solenoids performing the safety-related valve

venting function, in lieu of the valve positioners. If your understanding of

this commitment differs from ours, please contact me immediately. '

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

this letter and its enclosures will be placed in the NRC Public Document Room.  :
