SUBJECT:
NRC INSPECTION REPORT NO. 50-498/93-22; 50-499/93-22 (NOTICE OF
VIOLATION)
This refers to the inspection conducted by Mr Mark A. Satorius of this office
on June 28 through July 2, 1993.
The inspection included a review of
activities authorized for your South Texas Project, Units 1 and 2, facility.
At the conclusion of the inspection, the findings were discussed with you and
those members of your staff identified in the enclosed report.
Areas examined during the inspection are identified in the report.
Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observations of
activities in progress.
The purpose of the inspection was to determine
whether activities authorized by the license were conducted safely and in
accordance with NRC requirements.
Based on the results of this inspection, certain of your activities 2ppeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice).
The first violation, which contains two examples and
involves an event that occurred on June 13-14, 1993, is of concern because
both licensed and nonlicensed operators were determined to have failed to
follow station procedures in the performance of their watchstanding duties.
In the first example, licensed operators conducted an inadequate shift relief
because they failed to note that spent fuel pool cooling had been lost because
component cooling water had been isolated to the in-service spent fuel pool
cooling heat exchanger. The isolated component cooling water went unnoticed
in the control roon for over 13 hours1.50463e-4 days <br />0.00361 hours <br />2.149471e-5 weeks <br />4.9465e-6 months <br />, despite three indications being
available to alert operators of the abnormal condition.
In the second
example, a reactor plant operator did not conduct an adequate tour of the fuel
handling building because he f ailed to notice the significant reduction in
component cooling water flow noie.es following the isolation of cooling water
to the spent fuel pool heat exchanger.
This violation is indicative of significant operator performance weaknesses.
Because of the repetitive nature of this and similar cases involving

maintenance and operator performance problems that have been documented in
recent NRC inspection reports, a management mecting has been scheduled for
July 30,-1993, to discuss your corrective actions to address these personnel
i
performance problems and the actions you have taken or plan to take to
-
preclude recorrence.

corrective action program to take actions to ensure that a condition adverse
to quality is identified and steps taken to prevent recurrence. Specifically,
a similar event occurred during the second Unit 2 refueling outage where the
~
shifting of safety-related Class lE Distribution Panel DP002 from its normal
,
source of power to its secondary power source caused valves to change
!
position.
Following this previous occurrence, no station problem report or
other corrective action document was initiated to determine the root cause of
the event. Consequently, station personnel were not fully cognizant of the
!
potential plant changes that could be expected when shifting power supplies
for safety-related Class 1E Distribution Panel DP001 on June 13, 1993.

This violation was determined to be similar to previous violations concerning
your failure to take prompt and. effective actions to correc.t problems. We
understand Houston Lighting & Power Company is already taking significant
action to resolve weaknesses in its corrective action program as a result of
this and other NRC enforcement actions.
For this reason, no response to the
Notice is required.
