The purpose of this letter is to confirm verbal authorization provided to you on October 23, 1992, by John Eastman, of my staff, to commence work under the subject task order effective October 26, 1992 with a temporary ceiling of $20,000.00.

In accordance with Section G.5, Task Order Procedures, of the subject l

contract, this letter definitizes Task Order No. 21. This effort shall be i

l performed in accordance with the enclosed Statement of Work and TET, Inc.'s l

Technical proposal dated Ocotber 23, 1992, incorporated herein by reference.

l The period of performance for Task Order No. 21 is October 26, 1992 to j

November 13, 1992. The total cost ceiling is $21,061.94.

STATEMENT OF WORK FOR TASK ORDER NO. 21 UNDER FIN L-1474 CONTRACT NO. NRC 03-90-028

Title:

Independent Nondestructive Evaluations at LaSalle Nuclear Power Station Project Manager: Leta Brown (301) 504-1232 Technical Monitor: Michael C. Modes (215) 337-5198 Inspection Report No.: 92-024 NRR Priority No.: 3 BACKGROUND The NRC Mobile Nondestructive Evaluations (NDE) Lavoratory is used to perform independent nondestructive evaluations at nuclear facilities through out the United States. The independent evaluations are performed in order to verify the accuracy of the NDE performed by licensees and their vendors.

In order to perform independent radiographic analyses, the NRC must utilize the service of an organization that possesses a license to own and operate radioisotopic sources. These sources are used in the process of producing radiographs independently of the radiography performed by the licensee. The NRC does not own or operate such radiographic sources. In addition to the radiography performed, the technical personnel supplied by the contractor are utilized for other NDE during the inspection period. These inspections are to supplement the evaluations performed by the NRC.

OBJECTIVE The objective of this task order is to obtain the technical expertise of TET, Inc. to assist the staff in performing an independent assessment of the NDE performed at LaSalle Nuclear Power Station to ensure that the examinations, including liquid penetrant, magnetic particle, ultrasonic, radiography and visual examinations are performed in accordance with the requirements of the ASME B&PV Code.

TECHNICAL AND OTHER SPECIAL OUAUFICATIONS REOUIRED Provide two technicians on a full time basis for a period of 11 days. One of the two technicians shall be qualified to transpert and expose the source. The radiographer must also be qualified to expose i

film, develop film and inte.pret the radiographic results, using the mobile tmiler and NRC supplied equipme.nt and must be qualified currently as a Level II in radiography, liquid penetrant and magnetic particle to the requirements of ASNT: SNT-TC-1 A; 1984 Edition. It is preferable that this individual have five years or more of current experience. The radiographer shall be thoroughly trained in j

radiation safety practices as prescribed by 10CFR34 of the Code of Federal Regulations and shall be capable of making ASME Section III, V, and XI acceptable radiographs.

The second techrdelan shall be trained and qualified to perform ultrasonic, tragnetic particle, liquid penetrant and visual examinations. This individual should have a minimum qualification of Level II l

in conformance with the requirements of ASNT: SNT-TC-1 A,1984 Edition. It is preferable that this technician have a minimum of five years experience.

Both technicians shall have had a physical examination within the past year to include eye, urinalysis drug screen, pulmonary testing for respirator equipment and psychological screening. At least one of the technicians must possess a security clearance, or the equivalent, and must be capable of passing examinations required by the licensee for site access.

It is the responsibility of the contractor to ass!gn technical staff, employees, subcontractors, or specialists who have the required educational background, experience, or combination thereof to meet both the technical and regulatory objectives of the work specified in this statement of work. The NRC will rely on representations make by the contractor conceming the qualifications of the personnel assigned to this task order including assurance that all information contained in the technical and/or cost proposal, including resumes, is accurate and truthful.

OTHER APPLICABLE INFORMATION The work specified in this statement of work is license fee recoverable.

l The contractor will make the necessary arrangements to comply with the licensee insurance and i

liability requirements for site access of his employees. The contractor is responsible for compliance with local tax and employment regulations as well as any radiographic agreement state licensing and associated reciprocity fees.
