SUBJECT: NRC INSPECTION REPORT 50-445/93-11; 50-446/93-11

This refers to the inspection conducted by Mr. D. N. Graves, and the .

t

inspectors identified in the enclosed report, of this office, on January 31

through March 6, 1993. The inspection included a review of activities t

authorized for the Comanche Peak Steam Electric Station, Units 1 and 2,  ;

!

facility. At the conclusion of the inspection, the findings were discussed

with you and those members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within.  ;

-

these areas the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of i

activities in progress.

It was identified that, during a maintenance repair activity, inadequate i

procedural controls resulted in damage to a valve actuator housing in the

residual heat removal system. This occurrence was in violation of Criterion V  ;

=

of Appendix B to 10 CFR Part 50. However, this violation is not being cited

.

because the criteria in paragraph VII.B.2 of Appendix C to 10 CFR Part 2 of

the NRC's " Rules of Practice" were satisfied. This violation was an isolated i

occurrence and the licensee staff took prompt and effective actions to correct.
