c, ,o ? \\, NRC-10-87-171 Modification No. 21 Page 2 The purposes of this modification are to provide for: (1) the addition of the Wage Determination for Option Year 1 and Option Year 2 respectively; (2) increases to the total amounts for Items No. 2 and 3 for Option Year 1 and Option Year 2 respectively; (3) transfer funds available under Item No. 7 of Option Year 1 and Option Year 2 to provide for the increases resulting from the Wage Determina tions; (4) revise the Equipment Inventory List; (5) transfer funds available under Item No. 7 of Option Year 1, 2 and 3 to provide for the increases to the equipment inventory; and j (6) provide incremental funding and increase the ceiling of the contract. i Therefore, the following changes are hereby made to the contract. A. The Department of Labor Wage Determinations for Option Year 1 and Option Year 2 are hereby added to the Contract. Therefore, Section J, Item 3, Equipment Inventory List is revised to include the attached list of additional equipment. C. As a result of the above changes Section B.3.B Option Year 1 First Additional Twelve Month Period; Section B.3.C Option Year 2 Second Additional Twelve Month Period; and Section B.3.0 Third Additional Twelve Month Period are revised to: 1. Basic housekeeping. The homemaker uses devices normally found in private homes including brooms, light mops, dust cloths, household vacuum sweepers, and washig machines and dryers for laundry (laundry is done in the client's home j or at a laundromat). Specific cleaning tasks include sweeping, mopping and/or vacuuming floors, dusting f urniture, wiping counter tops and seove tops, def rost-ing ref rigerators, removing trash, and cleaning roetal and porcelain bathroom fixtures. Homemakers make beds and change bed linens, prepare and plan meals. s go grocery shopping and run errands for clients. Home management tasks for clients unable to manage the home because of serious illness or disability or because of temporary absense from the home. Spe ci f ic l i tasks may include planning and preparing meals, paying bills, scheduling time to insure that clients keep medical appointments, and maintaining the home. Child care, including emergency 24-hour care during the temporary absense of the ca retaxe r f rom the home. Child care may also be provided while the regular care-taker is in the home but needs assistance because of ' illness or disability or because children'are neglected and/or abused.
