which have contributed to the vialetion cited below.

Among the i

more important was a turnover of personnel and associated lack of continuity.

Nevertheless, San Diego State University is committed to maintaining an effective Radiation Safety Program which is comprehensive in scope-and will preclude any reoccurre,nce of the violation dier,ussed below.

contrary to License condition 14.C, no leak test records were maintained for leak tests conducted between December 31, 1987 and December 17, 1988, and between January 1, 1986 and November 30, 1986; and leak test records matutained from January 1,1985 through June 30, 1989 for 11 plutonium-beryllium sources contained no informatino describing test results and appropriate radioactivity units.

This is a repetitive violation of severity Level V.

1 Reason for Violation These violations occurred during the above mentioned periods because adequate procedures for documenting those specific activities had not been developed.

oc corrective steps Taken and Results Achieved A procedure to document '.eak testing was implemented in November, 1989.

Results of leak tests are recorded on a Leak Test Certifi-cate (See attached) using appropriate radioactivity units.

Corrective steps Taken to Avoid Further Violations The procedure used above (in conjunction with computerised record keeping) will preclude this ty;pe of violation from occurring in the future.

In addition, as men';ioned in the management conference call, we intend to obtain the services of a qualified individual (probably a Radiation Safety Officer from a sister CSU cam perform a thorough audit of our radiation safety program. pus) to Date when Full Compliance will be Achieved compliance with License condition 14.C has been achieved as of November, 1989.
