INSPECTION REPORT NO. 50-160/90-02 l

VIOLATION 50-160/90-02-01, SEVERITY LEVEL IV (SUPPLEMENT IV) l 10 CFR 20.203(c)(2) requires that-each entrance or access point to a high radiation area shall bet (i) Equipped with a control device which shall cause the level of radiation to be reduced below that at which an individual might receive a dose of 100 millirem in 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> upon entry into the area; or (ii) Equipped with a control I

device which shall energize a conspicuous visible or audible alarm i

signal in such a manner that the individual entering the high radiation area and the licensee or a supervisor of the activity. are made aware of the entry; or (iii) Maintained locked except during periods when access to the area is required, with positive control over each individual entry.

4 Contrary-to the above, on June 13, 1990, two graphite stringers, which were found to have a radiation level roading of 200 millirem per hour, were' located in a posted high radiation area which was l

not equipped with a control device to reduce the radiation level below 100 millirem in hour, nor equipped with a device - that energized a conspicuous visible or audible alarm signal, nor a

maintained locked.

REPLY 1.

Admission or denial of the violation:

The Georgia Institute of Technology agrees with the violation as stated.

The reason for.the violation:

The health physicist on duty at the time the stringers were removed from the reactor has a background in power o

plant health physics. The Technical Specifications for f

nuclear -power plants do not require locking High.

Radiation Areas until a radiation level of 1000 mrem / hour is reached.

Consequently, this individual was not aware t

that positive control of the high radiation area at 100 mrem / hour created by tM graphite stringers was required.

Corrective steps which have been taken and the results achieved.

The graphite stringers were immediately moved to an area which had walls, a cover, and a door which-was locked.

Additionally, the health physics staf f were informed that areas-in which the radiation levels were 100 mrem / hour or greater required positive control.

The options for positive control as provided by 10 CFR 20.203(c)(2) were reviewed for the health physics staff.

Corrective steps which will~ be taken. to avoid further violations:

6/28/90, was revised-to clarify the. requirement for positive control over a High Radiation. Area.-

The Nuclear Safeguards Committee approved the procedure revision on June 28, 1990.
