NRC-33-89-132 This modi 4 cation is issued to (1) increase the level of effort of this n
task order os described in the attached Statement of Work, (2) extend the task order period of performance through July 31,1991,(3) establish the total estimated ceiling amount for this extension at $46,139.26, (4) provide incremental funds of $30,000.00, and (5) reflect the key personnel change for the replacement of Kathy Robicheaux with Sharon Hire in accordance with an as accepted by the Contractor's May 8,1990 letter which is attached hereto and made a part hereof.
Therefore, the task order total estimated ceiling amount is increased by
$46,139.26 from $48,926.45 to $95,065.71 and the obligated amount is increased by $30,000.00 from $48,926,45 to $78,926.45.
Accordingly, the following changes are hereby made:

The third and forth paragraphs of the definitization letter for this task order are revised to read as follows:
" Task Order No, 04 shall be' in effect from June 16, 1989 through July 31, 1991 with a total cost ceiling of $95,065.71.
The amount of
$90,066.60 represents the total estimated reimbursable costs and the amount of $4,999.11 represents the fixed fee.
The amount presently obligated with respect to this task order is 578,926,45 of which $74,840.89 represents the total estimated reimbursable costs and the amount of $4,085.56 represents the fixed fee. This obligated amount may be unilaterally increased from time to time by the Contracting Officer by written modification to this task order. The obligated amount shall, at no time, exceed the task order ceiling. When and if the amount (s) paid and payable to the Contractor hereunder shall equal the obligated amount, the Contractor shall not be obligated to continue performance of the work unless and until the Contracting Officer shall increase the amount obligated with respect to this task order.

STATEMENT OF WORK Maintain.the common-User Guide Library by labeling record copies of each user guide, provided by the various branches of IRM, with its -corresponding three-ring binders, labeling the binders and placing the ' binders sequentially in a bookcase.
3, Maintain updated copies of all users and distribution lists -and the User Guide Inventory.
Receive updated pages to Users Guides, ensure that all pages are complete and accurate and that all references with the Table of Contents, appendices, are correct.
Ensure that all pages are. dated with the date of the update. : Reproduce sufficient copies utilizing NRC reproduction facilities and distribute. updates to the appropriate users. All updates shall contain a cover memo to the user identifying pages to be added, deleted or replaced. At no time shall guides or updates be sent to aryone not employed by the NRC without written permission of the NRC system project manager. The contractor shall not release any guides or updates without NRC contact approval. The contractor shall only use the NRC "in-house mail" system to distribute documents.
Evaluate all existing record ccpies of Users Guides to ensure completeness of identified contents and existence of ey.hibits and appendices.
Prepare a brief description of proposed upgrades for those guides deemed to be deficient. Submit this list of upgrades to the Task Manger.
Maintain at least two copies of each User Guide and its updates.
Maintain two copies of the user Guide Library Diskettes, containing the final documents. One, working copy, will reside with the contractor and a back-up copy with-the Task Manager. A Partitioned Data Set shall contain the final. documents and be stored at the NIH/DCRT facility.
Maintain an archive of all final record copies for old, deleted, inactive or non-operational systems.
The level of effort for this task order is estimated to be 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br />.
