This modification is issued to reflect-(1) the increase of the level of-

granted by the NRC Contracting Officer which (a) extends the tasx order-period of performance through May 31, 1991; (b) establishes the total estimated ceiling amount for this extension at $23,378.96, and (c) provides incremental funds of $23,000,00.

J The. contractor.shall perform the data entry. work:at the NRC site in Bethesda,.

7 entry is presented.in Attachment II (the data element nanes that will probably-

r'equire entry.have been circled).. There are approximately 120 to.140 LERs1to-g

>beLupdated each week and will1 consist-of approximately 10 data elements.plus-4 f,~

text (the abstract): averaging about'15 eighty character lines.

Simple tinstructions and training will be provided to the data entry: person at the NRC:

t l site. After an initial brief break-in period,- it,is anticipated that'each document will take about 5-- 10 minutes.to, complete. This is expected to'be a

On December 12 1985 at 6:37 AM a reactor t rip occurred due to loss of reactor coolant flow signals from reactor coolant loop 821.

statiori's 6,9 KV buses was disrupted atthe normal supply of offsite 138 KV powerDuring recovery activities, to the 8:00 AM.

Offsite power was reconnected at 8:20 AM.

Concurrently, Auxiliary Feedwater Pump were experienced andseveral automat ic trips of 823 the pumo was declarec inoperabie'at 10:30 AM. -The pump was returned to operable status at 5.25 g

PM.

The cause of the low flow si'nal was a failed bistable in one instrument 9

channel, coincident with a signal from a separate low flow instrument-channel, which had been previously placed in the tripped position.

The interruption in of f sit e power was due to physical tapping of a relay box which actuated a' relay.

The loss of the auxiliary f eedwa t e r p ump wa s due to a failed relay which was replaced.
