Modification No. 2 to Task Order No. 02 Under Contract No, NRC-39-89-133 This modification is issued to reflect (1) the increase of the 1e.el of effort of this task order as described in the attached Statement of Work and (2) the confirmation of the verbal authorization of May-30, 1990 granted by the NRC Contracting Officer which, effective May 31, 1990, (a) extends the task order period of performance through July 31,1991; (b) establishes the total estimated ceiling amount for this extension st-
$21,580. 61; and (c) provides incremental funds of $21,580.61.
Therefore, the task order total estimated ceiling and obligated amounts are increased by $21,580.61 from $16,782.00 to $38,362.61.
Accordingly, the following changes are hereby made:
1.
The' third paragraph of the definitization letter for this task order is revised to read as follows:
" Task Order No. 02 shall be in effect from February 1, 1989 through July 31, 1991 with a total cost ceiling of $38,362.61.
The amount of
$35,855.47 represents the total estimated reimbursable costr and amount of $2,507.14 represents the fixed fee.
The total cost ceiling amount of $38,362.61 includes the $21,530.61 verbally authorized to

The contractor shall provide data entry support for the purpose of entering NRC training records into the Agency Training System.
The source of information comes from several documents:
NRC FORM 368 - Training Request and Authorization Approx. 125 per week NRC FORM 121 - In-house Training Registration Card Approx. 175 per week In-house Training -Training Request Form Approx. 100 per wech, and A Hard-copy Printout of data from several NRC PC-based Training systems.
Upon completion of entering this data the contractor shall verify the accuracy of the data by reviewing several proof reports and make corrections where.necessary.
Worh shall be performed using a IBM PC, located in the Woodmont Building. Bethesda, Md four (4) days a week 3
