i APPROVAL RECORD Model No. B-2 Package Certificate of Compliance No. 6144 Revision 11 By application dated December 6, 1989, as supplemented on May 2, 1990. US-Ecology, Inc, requested that Certificate of Compliance No. 6144 for the Model No. B-2 package be renewed for a five year period.
No changes to the package design have been requested since the certificate was renewed on January 22, 1985.
The certificate has been conditioned to require that the package be prepared for shipment, operated, and maintained in accordance with US Ecology, Inc. Procedure CSK-1, Rev. O, dated May 2, 1990.
The certificate has been renewed for a five year term which expires June 30, 1995.
