Dear Mr. Dickherber:
The enclosed Order is being issued as a consequence of events occurring during '
refueling operations at.the Quad Cities Nuclear Power Station, Unit 1 on October 17, 1989. The Order immediately suspends your Senior Operator License
'imited to Fuel Handling (License) and requires you to show cause why your ,
License should not be revoked. This action is being taken because of your -
willful violation of Comission requirements. We recognize the limited safety significance of the actual fuel move. However, improper movement of fuel could result in an lnadvertent criticality and is unacceptable.
I In accordance with Section 2.790 of the NRC's " Rules of Practice", Part 2 Title 10, Code of Federal Regulations, a copy of this letter and the enclosures will be placed in the NRC's Public Document Room.
