ORDER SUSPENDING LICENSE (EFFECTIVE IP94EDIATELY) AND ORDER TO SHOW CAUSE WHY LICENSE SHOULD NOT BE REVOKED
Dear Mr. Dickherber:
The enclosed Order is being issued as a consequence of events occurring during refueling operations at.the Quad Cities Nuclear Power Station, Unit 1 on October 17, 1989.
The Order immediately suspends your Senior Operator License
'imited to Fuel Handling (License) and requires you to show cause why your License should not be revoked. This action is being taken because of your willful violation of Comission requirements. We recognize the limited safety significance of the actual fuel move. However, improper movement of fuel could result in an lnadvertent criticality and is unacceptable.
I In accordance with Section 2.790 of the NRC's " Rules of Practice", Part 2 Title 10, Code of Federal Regulations, a copy of this letter and the enclosures will be placed in the NRC's Public Document Room.
