STATUS OF PREVIOUS INSPECTION FINDINGS:

1.

(Closed) Deviation (79-01):

The QA program did not address the requirements for QA records.

QAP number 4 has been issued which addresses the requirements for QA records.

2.

(Closed) Nonconformance (81-01):

Records were not maintianed as required by applicable specific contract requirements such as OPS-386-2.

A contract change notice has been issued to waive the requirements of OPS-386-2 which were not maintained.

3.

(Closed) Nonconformance (81-01):

Orawings were not controlled in that two out-of-date revisions were found and a master drawing list was found to contain two entries with erroneous revision levels.

A review of a current job in process found no further problems in this area.

Drafting has issued a memo which addresses prevention of this problem.

4.

(Closed) Nonconformance (81-01):

Sampling inspection was not defined in procedures.

Quality Control Instruction number 135 has been revised to document the use of MIL-STD-105.

5.

(Closed) Nonconformance (81-01):

The sequence of calibration operations precluded the identification of some "out-of-calibration tools."

The calibration procedure QCI-20 has been revised and issued to address the problem identified.

OTHER FINDINGS OR COMMENTS:

1.

Test Controls - The functional testing of three recent orders was reviewed for jobs 02YN163, OlYN093, and OlYN094.

The test logs were inspected and the test procedures reviewed.

The customer requirements and procedures were found to be implemented with the exception noted in the followup item (below).

Insulation, polarity, sequence, etc.,

were the type tests being performed.

The use of approved drawings and procedures was verified.

The documentation and disposition of defects found during testing were verified to follow established procedures.

Followup Item - It was noted that the seismic testing of assemblies for jobs 01YN093 and 01YN094 did not have steps 5.2 to 5.4 of its special testing instructions completed, although the units were released.

The customer approval of the special testing instructions and the standard testing appears to have been waived; however, the documentation of this is confusing.

Engineering is to pick up the missed test steps prior to seismic testing, and the test data is to be reviewed and approved by the customer.

The above will be verified during the next inspection with a general review of test control measures.

3.

Control of Special Processes - The standard processes of plating, welding of cabinets, and coil impregnation were inspected.

These processes are used for both nuclear and nonnuclear applications.

Parts of these points in fabrication are generally not designated for their final application.

Conformance to procedures and specifications was verified.

Analysis of plating tanks was verified.

Qualification of welding personnel was verified.

The viscosity checks of the varnish used for impregnation were verified.

The inspection procedure on coil impregnation QCI-100, should be revised to document current practices.
