APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket Nos. 50-245 50-336 Millstone Nuclear Power Station License Nos. DPR-21' DPR-65 As a result of the inspection conducted on June 1 - 4, 1982, and in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C), 47 FR 9987 (March 9, 1982), the following violation was identified.
10 CFR Part 50, Appendix B, Criterion V, states, in part, " Activities affecting quality... shall be accomplished in accordance with Instructions, Procedures, or drawings...."
Section 3.2.1 of the Northeast Utility Quality Assurance Program Topical Report (Rev. 4a) states, in part, " Design Control Measures require that applicable design requirements... are translated into specifications, drawings, procedures, or instruction."
Section 6.1 of the Northeast Utility's Quality Assurance Program Topical Report states, in part, " Approved controlled documents are distributed to affected locations in accordance with controlled distribution lists.
Changes to controlled documents are reviewed and approved by the same organization which performed the original review and approval."
Section 6.14.2 of Station Administrative Control Procedure No. ACP-QA-3.04 (Revision 9) states, in part, "A significant change to an approved PDCR will be accorded the same review process as the original PDCR."
Contrary to the above, as of June 4, 1982:
(1) An audit sample of seven Plant Design Change Requests (PDCR's) identified that these PDCR's did not contain the required documentation to demonstrate how the applicable design requirements were translated into drawings and other design documents.
These PDCR's were:
2-86-80 1-113-79 2-117-80 1-120-79 2-80-001 1-19-79 1-20-79 (2) Approved and controlled drawings for two completed PDCR's (PDCR 1-113-79 and PDCR 1-19-79) were not distributed to the affected locations in accordance with controlled distribution lists.

(3) An approved PDCR (PDCR 2-41-81) was significantly changed, after its approval, to use two Model 3 snubbers instead of one Model 10 snubber as approved, without a review of the change.
The above collectively constitutes a Severity Level V Niolatior.
Pursuant to the provisions of 10 CFR 2.201,-Northeast Nuclear Energy Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
