LASALLE COUNTY STATION:

Unit 1 Ill. EVENT DESCRIPTION The Regenerative Wastes Solution Discharge Tank (OWM03T) was discharged on 5/26/82 and 6/5/82.

Both of these discharges exceeded the N.P.D.E.S.

Permi t ILOO48151 maximum discharge limit for low volume wastes (Outfall 001c) of 1.0 mg/l total iron. Analyses of composite samples for these two discharges yielded a total Iron concentration of 1.1 mg/l and 2.2 mg/1, respectively.

These noncompliances are reportable under Tech Specs Appendix B, Section 3.2.

IV.

PROBABLE CONSEQUENCES OF THE OCCURRENCE The health and safety of the general public was not affected by these occurrences.

Excursions of the total iron limit is a continuing problem for this outfall, until FCR # LOO 72 modifications are completed.

V.

CAUSE On 5/25/82 dirt and debris from maintenance work that had been performed in the area was hosed into floor drains. These drains are routed to the Regen. Vastes Soln. Disch. Tank.

Since the dirt and debris contained rust, the composite sample of the tank exceeded the Permit limit.

The composite sample analyzed on 6/5/82 excursion was due to the temporary injection system.for hydrogen peroxide being ai r-bound.

Thus, the hydrogen peroxidt_(H 0 ) solution was not injected into the raw well water. Without 22 the injection of H O soluble iron in the raw well water is not oxidized to aninsolubleforman$,removedbyfiltration: This soluble Iron was sub-2 sequently removed by ion exchange and discharged to this outfall.

VI.

CORRECTIVE ACTION The operators have been instructed to discontinue washing excessive dirt and debris down floor drains, especially ones routed to N.P.D.E.S. outfalls.

These floor drains have also been appropriately marked for identification.

The air-bound temporary injection system was returned to routine operation, and operators have been instructed to monitor the operation of the system better.

Also, a permanent H O injection system is to be installed through the completion of FCR LOO 72 anbv1-82-301, assigned to Project Engineering.
