Holyoak letter to J.
G. Keppler da te d Jul y 7, 1982, Special Report.
Dear Mr. Keppler:
C.
W. Schroeder and D.
L. Shamblin o f Commonwealth Edison Company notified Mr. R. D. Walker of your staff, on June 9, 198 2, o f a possible reportable deficiency on certain fire dampers provided by Zack Company.
That notification was made in accordance with 10 CFR 50.55(e) for Unit 2.
Re ference (a) was submitted to you for LaSalle County Station Unit 1.
Description of De ficiency On Ju ne 8, 1982, Zack Company informed Commonwealth Edison Company of a possible problem with 42 Unit 1 and Unit 2 safety related fire dampers having a blade width o f 18 inches or less.
Apparently, the manufacturer's 21/2 inch fuse link assembly was not wide enough, causing the clinch straps to be held tight against the blade stack.
This may prevent the blades from dropping freely.
Corrective Action In order to correct the possible deficiency, the manufacturer recommended installing a 3 inch fuse link assembly.
This will prevent the clinch strap from interfering with the blade stack.
On Ju ne 9, 1982, clover hooks for the 3 inch fuse link assemblies were ordered by the Zack Company.
These arrived at LaSalle County Station on June 28, 1982.
Installation on all 42 Unit 1 and Unit 2 af fected safety related fire dampers was completed on July 1, 1982.
