SCOPE:
This inspection was conducted as a result of the issuance of a T0 CFR Part 21 report pertaining to improper application of a Westinghouse Type ECT current transformer in the parelleling circuit of emergency diesel generator static exciter voltage regulator subsystems.
The report indicated that the deficient hardware had been supplied to the following nuclear power stations:

Additionally, the following programmatic areas were inspected: (1) initial management meeting; (2) initial quality assurance program; (3) manufacturing process control; (4) change control; and (5) implementation of 10 CFR Part 21.
A.
VIOLATIONS:
Contrary to Sections 21.6 and 21.51(b) of 10 CFR Part 21:
a.
Posting of 10 CFR Part 21, Section 206 of the Energy Reorganization Act of 1974, adopted procedures, or an appropriate notice had not been accomplished.
b.
Records of evaluation had not been prepared for the current transformer that failed to meet the specification.
B.
NONCONFORMANCES:
1.
Contrary to Criterion V of Appendix B to 10 CFR Part 50 and paragraph 7.5 of Section 7, dated June 24, 1981, of the Quality Assurance Manual (QAM), documentary evidence that the Prototype Transformer, Part No. CD-1408, conformed to procurement requirements, drawings, and specifications had not accompanied the item; nor had a shortage form been initiated and the item retained in receiving.
This prototype transformer is the replace-ment for the noncompliant Westinghouse current transformer.
2.
Contrary to Criterion V of Appendix B to 10 CFR Part 50, and paragraph 10.3 of Section 10, dated June 24, 1981, of the QAM:
a.
Inspection operations / instructions had not been reflected on the shop traveler or job tickets for Manufacturing Order Traveler No. (17) 200 18107512-513; Drawing Nos. B-69294-5, CDL-11183; Stator Coils.

Contrary to Criterion V of Appendix B to 10 CFR Part 50, and Inter-Company Correspondence, dated February 4, 1982, regarding Manufacturing Order Traveler No. (2) 100 17707372; Drawing Nos.
D-67453E "W", C-34231A-4; Stator Punching:
a.
The approval to run space of the Punch Press Quality Assurance Log (PPQAL) had not been signed and dated despite completion of verifying the adequacy of the first article and the opera-tion's having started.
b.
Theperiodwhentheoperationwasnotrunninghadnotbeen indicated on the PPQAL despite the operation s being stopped due to equipment problems.
4.
Contrary to Criterion V of Appendix B to 10 CFR Part 50, and paragraphs 6.3 and 6.4 of Section 6, dated June 24, 1981, of the QAM:
a.
The following revised product travelers had not been reviewed and approved by the Manufacturing Manager, prior to distribution, as evidenced by the lack of signature or other indication; Manufacturing Order Travelers Nos.:
(19) i 200 18107512-513, dated January 15, 1982; Drawing Nos. B-69294-5, WDL 11183; and (17)200 18107512-513, dated January 14, 1982; Drawing Nos. B-69294-5, CDL 11183.
b.
The latest revisions to the following QA documents had not been reviewed and approved by the Quality Assurance Manager, prior to distribution, as evidenced by approval signatures that predate the revisions; EQ Nos..
2.2.2, dated May 16, 1978; 2.2.4, dated June 7, 1978; 2.5.18, dated June 5, 1979; and 2.12, dated February 12, 1979.
It was noted that Section 6, dated August 2, 1977, also contained the identical requirement.

UNRESOLVED ITEM:
The inspector observed that Drawing No. A-48048-21, Revision 3, dated January 27, 1982, was in use in the Small Stacking Area, to accomplish the task identified on Manufacturing Order Traveler No. (15) 600 1812522.
The drawing had been marked with a red felt-tip pen in several locations with no indication of the person responsible for the marking.
Subsequent to inquiry, the inspector was informed that there was no procedure addressing marked prints.
As a result, the inspector could not determine the validity of using marked prints.
D.
OTHER FINDINGS OR COMMENTS:
1.
Follow up on a Request from Headquarters - The request was initiated as a result of a 10 CFR Part 21 report that related to improper application of a current transformer, Westinghouse Type ECT.
Improper application was the result of selecting a transformer with appropriate current rating without giving sufficient consideration to the ambient temperature at the location of its installation.
Inadequate design verification compounded the problem.
Planned replacement of the noncompliant transformers is satisfactory.
The transformer has been redesigned for use at the ambient temperature of installation location and another source of supply has been selected.
A prototype has been received and subjected to a performance test which is the first of a series of planned tests.
Documentation of activities associated with the prototype transformer did not provide assurance that design control was adequate; i.e., (1) the device did not go through receiving inspection, resulting in a lack of documen-tation,' (2) performance test data did not reflect a review for acceptance analysis and evaluation, and (3) performance test data did not provide accept / reject criteria.
Additionally, it was determined that records of evaluation had not been prepared.
As a result of the foregoing, one violation and two nonconformances were identified.
2.
Programmatic Areas Inspected - This area of the inspection related to:
(1) initial management meeting; (2) initial quality assurance program; (3) manufacturing process control; (4) change control; and (5) imple-mentation of 10 CFR Part 21.
This activity resulted in one violation and three nonconformances.
While only four QA documents (EQ Specifications) were identified in the nonconformance, the majority were in that category.

The QA Manager went to work for the company on January 3, 1982.
The company also furnishes pump motors for nuclear generating stations.
It could not be determined that they were for Class IE application.
