5/27/82 -
At 2155 the Unit power was reduced to 85% to perform Secondary System Flow Verification and Heater Drain Pump Tests. Unit power was returned to 100% at 1212 on May 28, 1982.

Svensson TELEPHONE (616) 465-5901 PAGE 1 of 1 MAJOR SAFETY-RELATED MAINTENANCE MAY, 1982 M-1 The 480 volt circuit breaker for pressurizer heaters, Nos. 11, 12 and 35 tripped and would not reset. Replaced the circuit breaker.
C&I-l All alarms on Control Room annunciator panel No. 21 were illuminated and would not reset, apparently due to damage caused by a lightning strike. Annunciator printed circuit cards 1, 30 and 37 were replaced with spare cards obtained from the annunciator panel itself.
Panel No. 21 annunciators then were reset and functioned correctly.
C&I-2 Steam generator No. 2 power-operated relief valve (PORV), MRV-223, was leaking-by. The valve was first verified to be operating and seating properly. The controlling electrical-to-air pressure transducer, EPT-223, was found to have a high air output pressure which was causing MRV-223 to remain slightly open at all times. After recal-ibration of the EPT, no leakage from the MRV could be detected. A l
similar problem was corrected on steam generator No. 4 PORV, MRV-243.
C&I-3 Steam generator No. 3 power-operated relief valve, MRV-233, was leaking-by.
Investigation disclosed that deterioration of the valve's posi-tioner and booster relays had occurred, evidently caused by the extreme temperature environment.
Both positioner relays were replaced, and the booster relays were reconditioned. After recalibration of control transducer, EPT-233, proper operation and seating of MRV-233 was verified.
C&I-4 Reactor coolant pump No. 4 loop flow low alarm was received briefly.
Alarm module, FB-455A, manufactured by Foxboro Corporation, had ex-perienced a high drift of its setpoint. The module was recalibrated, but a subsequent check disclosed that the setpoint had now drifted low. A new module was installed and calibrated.
Several hours later the setpoint was verified to be within specification with no tendency to drift.
C&I-5 Diesel generator "CD" jacket. water surge tank makeup valve failed open.
Investigation revealed cracks in the valve's float which allowed the float to fill with water.
The float was replaced and proper operation of the surge tank inlet valve was verified.
