Appendix NOTICE OF VIOLATION Public Service Indiana Docket No. 50-546 Docket No. 50-547 As a result of the inspection conducted on May 1 through 31, 1982, and in accordance with NRC Enforcement Policy, 47 FR 9987 (March 9, 1982),

the following violation was identified:

10 CFR 50, Appendix B, Criterion XII Control of Measuring and Test Equipment, states " Measures shall be established to assure that tools, gages, instruments, and other measuring and testing devices used in activities affecting quality are properly controlled, calibrated, and adjusted at specified periods to maintain accuracy within necessary limits."

Public Service Indiana's Marble Hill PSAR Chapter 17 dated April 18, 1980 states in part, "The PSI quality assurance specification requires that contracting organizations performing quality-related activities requiring use of measuring and test equipment establish measures to assure that tools, gauges, instruments, and other measuring and testing devices are of the proper type, range and accuracy for their application."

Newberg-Marble Hill Procedure QCP 9.01, AWS Welding Inspection, l Section 12.2, required use of a " Calibrated device to check weld rod I

storage ovens monthly".

Contrary to the above, the contractor failed to use a calibrated device (thermometer or pyrometer) to perform these checks for several months.

A temperature indicating crayon (tempil stick) was utilized to verify this temperature. Use of these crayons is not acceptable as a calibrated device because unique traceability or correct accuracy cannot be provided.

This is a Severity Level V violation (Supplement II).

The inspection showed that action had been taken to correct the identified item of noncompliance and to prevent recurrence. Consequently, no reply to this item of noncompliance is required and we have no further questions regarding this matter.
