REPLY TO APPENDIX A OF NRC INSPECTION REPORT 50-317/82-07; 50-318/82-07 Item 1 Calvert Cliffs Instruction ll2C, " Safety and Safety Tagging," requires that af ter the completion of a job, a Tagging Authority person shall remove the tag (s), reposition the equipment to the cleared tag position, initial and date the tag-out record, and instruct another person to inspect the equipment and ensure it is in the proper position. On May 1,1982, a tag on the handswitch for //22 service water pump was removed and the equipment restored to its cleared position without the required documentation by the Tagging Authority and without indication that an independent check was performed until May 5,1982.
Clearing of the tags and the physical verification were performed in accordance with the procedure.
The cause of the event was the failure of Tagging Authority personnel to document the restoration of //22 Service Water Pump and its associated verification. To ensure that this event will not recur, CCI-112C and CCI-307, " Operations Shift Turnover Checklist,"
will be modified to provide a means for a Senior Reactor Operator to perform and document a daily review of the documentation associated with tag-outs initiated and closed out during the previous day. This change will be in effect prior to August 1,1982.
