CHANGE pc_} DATE n c 'f hf o o 6.0 SYMPTOMS 6.1 Unevaluated general area radiation levels exceeding 2.5 mrem /hr out-side of the controlled access areas which are not attributed to any cause other than a possible radiological incident.

6.2 Unevaluated airborne radioactivity in excess of 1 x 10~ pCi/cc mea-sured outside of the controlled access areas.

6.3 An uncontrolled toxic gas leak where the hazard is not confined to a local area and the conditions continue to increase in severity.

7.0 IMMEDIATE ACTIONS 7.1 Any person who identifies any of the emergency conditions described in Section 6.0 shall notify the affected unit's Shift Operations Supervisor immediately. If the potential for a plant evacuation exists and the Emergency Response Organization has been activated, the Shif t Operations Supervisor and the Duty Emergency Coordinator /

Recovery Manager shall discuss whether or not conditions warrant a plant evacuation.

7.2 Upon verification, the Shift Operations Supervisor shall contact, by the most expedient means, the following groups, as necessary, and pro-vide them with designated assembly area, evacuation route (s) (based upon symptoms and prevailing wind direction) and protective measures, as necessary: .

7.2.1 Main Guard Station 7.2.2 Emergency Evacuation Team NOTE: The evacuation announcement should include evacuation routes and designated assembly area (this announcement should be repeated several times, if possible).

7.3 The Shif t Operations Supervisor shall declare a Plant Evacuation over the Public Address System and sound the plant evacuation alarm.

7.4 Shif t Operations Personnel on duty should report to the Control Room.

8.0 FOLLOW UP ACTIONS 8.1 Determine if any Emergency Action Level criteria have been exceeded.

(Refer to 1903.10, " Emergency Action Level Respor.se").

8.2 Shift Operations personnel which are onsite but not on duty should report to the Control Room from the appropriate response center.

8.3 Personnel other than Shift Operations Personnel in the Controlled Access Areas shall proceed to the Controlled Access points and monitor themselves, if possible, prior to exiting.

5.3 Security Personnel are responsible for evacuation and assessing per-sonnel accountability following an Exclusion Area Evacuation.

5.4 The Emergency Evacuation Team is responsible for assisting Security personnel as directed in evacuation and personnel accountability.

5.5 The Emergency Medical Team is responsible for decontaminating and aiding injured contaminated personnel identified during an Exclusion Area Evacuation. -

5.6 Emergency Team Leaders are responsible for reporting, accountability of team personnel to the Technical Support Center A g _,,. A 6.0 SYMPTOMS 6.1 An Exclusion Area Evacuation will be initiated if it is determined that personnel within the Exclusion Area could:

6.1.1 Receive a whole body dose of 0.5 Rem or a thyroid dose of 1.0 Rem (over a period of 12 months as a result of the incident).

6.1.2 Receive an exposure to a toxic gas.

7.0 IMMEDIATE ACTIONS 7.1 Any person who identif'ies any of the emergency conditions described in Section 6.0 should notify the affected unit's Shift Operations Supervisor. If the potential for a plant evacuation exists and the Emergency Response Organization has been activated, the Shift Operations Supervisor and the Duty Emergency Coordinator / Recovery Manager shall discuss whether or not conditions warrant an exclusion area evacuation.

7.0 INSTRUCTIONS 7.1 If the projected dose rate beyond the exclusion area is SmR/hr or greater whole body, or 25 mR/hr or greater thyroid then the appropriate individual (the highest level of authority present) i.e.

Incident Response Director Recovery Manager Duty Emergency Coordinator Shif t Operations Supervisor should take the following actions:

7.1.1 Notify the Arkansas Department of Health or the Arkansas Nuclear Planning & Response Program (per 1903.10, "Emer-gency Action Level Response") of existing plant condi-tions and projected dose rates. No protective action recommendation is necessary at this level unless the situation dictates otherwise.

7.1.2 If the agencies in Step 7.1.1 cannot be reached, the Sheriff (s) of the affected county (ies) shall be con-tacted (refer to 1903.10, " Emergency Action Level Re-sponse", Attachment 9 for phone numbers). Provide the information from Step 7.1.1.

7.2 If dose projections based on current dose rates under actual meterological conditions and estimated time of exposure project whole body doses of 1 R or greater or thyroid doses of 5 R or greater:

7.2.1 Notify the Arkansas Department of Health or the Arkansas Nuclear Planning and Response Program (per 1903.10,

" Emergency Action Level Response") of the existing plant conditions and the projected doses. A protective action recommendation shall be made to evacuate and/or shelter the affected sectors.

7.2.2 If the agencies in Step 7.2.1 cannot be reached, the Sheriff (s) of the affected county (ies) shall be con-tacted (refer to 1903.10, " Emergency Action Level Re-sponse", Attachment 9 for phone numbers). Provide the information from Step 7.2.1.

8.0 If during an area evacuation, the plant and/or exclusion area is required to be evacuated, the evacuation and accountability shall be accomplished per 1903.30, " Plant Evacuation" or procedure 1903.31, " Exclusion Area Evacuation".
