Box 968 Richland, Washington 99352 As a result of the inspection conducted on April 1-30, 1982 and in accordance with the flRC Enforcement Folicy 10 CFR Part 2 Appendix C 47 FR 9987 (March 9, 1982) the following violations were identified:
10 CFR 50 Appendix B Criterion V, as addressed in paragraph 17.4.5 of the Quality
" Activities affecting Assurance Program in Section 17 of the PSAR, states in part:
quality shall be prescribed by docunented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings..."
Detail B-B of the project design drawing F435860 requires a partial a.
penetration weld of 3/4-inch size, for the Tee-joint connection of nenbers
- 2 and #3 of pipe rupture restraint MSS-Pil-3-2.
Contrary to the above, on April 19, 1982 the completed, inspected, and accepted restraint MSS-Pil-3-2, furnished by Corner and Lada Company, contained a partial penetration weld of 7/16-inch size along 10-inches of the accessible length of one Tee-ioint weld.
This is a Severity Level V Violation (Supplement II), applicable to Unit 1.

Foley-Wisner-Becker procedure QCP-5A requires that "All welds will be docunented on Ueld Inspection records". This procedure was reviewed and approved by the licensee's representative, in accordance with the quality assurance progran.
It provides reference to weld inspection criteria.
Contrary to the above, in nid-February the inspectors for Foley-Wismer-Becker ceased to prepare the required records, and had alternatively instituted practices of an internal instruction OCI-9B. The required Ueld Inspection Sheets were not prepared for conduit suoports 1C 30!!-3418-ADA and IC 30H-3418-ADB.
This is a Severity Level V Violation (Supplenent II), applicable to Unit 1.
The Preliminary Safety Analysis Report Section 3.12 states that quality c.
assurance reouirenents for storage and handling of itens for UNP-1 will confom to the regulatory position of Regulatory Guide 1.38, which involves ANSI-f!45.2.2-1972. The inplementing Bechtel ifMP-1/4 Nuclear Quality Assurance Manual Section IV flumber 2 requires that " Receiving, controlling, storing, naintaining of material and equipnent meet the apolicable requirements of applicable drawings and specifications, supplier's requirenents and A!!SI-fl45.2.2". The A!!SI-fl45.2.2 Parts 1.2 l
and 6.5 require protection of couipnent from deterioration durino storage and installation.

o-The Equipment Maintenance procedure llP/P-4 Section A13.1 requires that Bechtel " Implement the Project Preventive Maintenance Program on all stored equiprent." and
"...on equipment in the site Contractor's custody."
Contrary to the above, the preventive maintenance program was not fully implemented on all equipment in the contractors' custody, such that:
(1) By March 1982 the protective neasures had been insufficient to prevent rusting and physical damage to machined flanqe faces on containnent encapsulation vessels DHR-VSL-1A and 2B for decay heat renoval systen valves. This equipment was in custody of the J. A. Jones Company.
(2) By April 1982 the protective neasures had been insufficient to prevent accumulation of metal shavings and other fine debris in 13800/480 volt switchgear identified as 1-APN-USS-0EB1-B. This equipment was in the custody of the Foley-llismer-Becker Connany.
This is a Severity Level V Violation (Sunnlenant II), applicable to Unit 1.
Pursuant to the provisions of 10 CFR 2.201, llashington Public Power Supply System is hereby required to submit to this office within thirty days of the date of this notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full conn 11ance will be achieved.
Consideration may be given to extending your response time for good cause shown.
