Appendix NOTICE OF VIOLATION Union Electric Company Docket No. 50-483 As a result of the inspection conducted on April 26 to May 7,1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9,1982), the following violation was identified:

10 CFR 50, Appendix B, Criterion XVI states, in part, " Measures shall be established to assure that conditions adverse to quality such as failures, malfunctions, deficiencies, deviations, defective material and equipment and nonconformances are promptly identified and corrected. In case of significant conditions adverse to quality, the measures shall assure that the cause of the condition is determined and corrective action taken to preclude repetition."

The SNUPPS Quality Assurance Program in Section 17.1.16 states, in part, "The SNUPPS utilities have established a corrective action program which determines the need for corrective action and assures that conditions adverse to quality are promptly identified, reported, corrected and that corrective action is appropriate and effective. The program requires that the cause of adverse conditions be determined, corrective action be taken to preclude repetition and~ verification be made that these actions have been impicmented in a timely manner in accordance with specified requirements."

Contrary to the above:

1. Deficiency reports identifying damaged cables in Cabinet RJ160A in the Auxiliary Building were dispositioned in December 1981, on the basis of-corrective-action involving the training of craft personnel. The in-spector found that prompt corrective action to preclude repetition was not taken in that, as of May 7, 1982, no documented training had been given to the craft personnel.
2. A deficiency report identified, in September 1981, that field installed cables in main control pancis violated the minimum separation criteria.

The inspector found that, as of May 5, 1982, the cause of the adverse condition had not been determined, in that neither licensee nor con-tractor personnel had identified that the problem was related to the physical routing of cables belonging to Safety Groups 1 and 4 and also non-safety related cables.

3. A deficiency report identified, on February 4, 1982, that eight safety-related cables had been terminated with incorrect lug hardware. The action cited to prevent recurrence was to clarify, for the field, the material to be used. A subsequent deficiency report on April 6, 1982, noted that additional safety-related cables had been terminated with the incorrect lug hardware. The inspector found that prompt corrective action to preclude repetition was not taken, in that terminations with the incorrect lug hardware continued to be made for two months after the initial deficiency report on the subject had been issued.
4. Twelve specific audit findings identified by Union Electric QA in 1981 audit reports had been responded to in a timely manner by the construc-tor, Daniel International. The inspector found that timely corrective actions had not been taken by UE QA in that, as of May 7, 1982, only eight of the responses had been reviewed, and none had been closed out.

This is a Severity Level IV violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
