APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket No. 50-423 Millstone Nuclear Power Station Unit 3 License No. CPPR-113 As a result of the inspection conducted April 12-30 and May 4-14,1982, and in accordance with the NRC Enforcement Policy (10 CFR Part 2 Appendix C),
47 FR 9987 (March 9,1982), the following violation was identified:
10 CFR 50, Appendix B, Criterion V and Northeast Utilities QA Pmgram Topical Report, Section 5, states that activities affecting quality shall be accomplished in accordance with (the prescribed) instructions, pmcedures, or drawings.
Stone and Webster Quality Standards QS-10.31, states in section 4.1.2.A that the construction department is responsible for installing mechanical equipment and components in accordance with the equipment installation speci-fications, drawings, and manufacturer installation instructions.
Contrary to the above, as of May 4,1982, four Mechanical Pipe Snubbers were installed in the Primary Containment without following all the manufactumr's instructions.
This is a Severity Level V Violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Conpany is hereby required to submit to this office within thirty days of the date 2)(1) of this Notice, a written statement or explanation in reply, includi the corrective steps which have been taken and the results achieved; correc-tive steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affimation. Where good cause is shown, consideration will be given to extending your response time.
The msponses directed by this Notice are not subject to the clearance proce-dures of the Office of lianagement and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
