Due to temporary failure of peroxide injection pump.

Peroxide pump had lost suction pressure due to a bend in feed hosing, which was straightened out, and later replaced. This is in excess of NPDES permit limit for total i ron of 1.0 mg/l and it reportable under Tech. Spec. Appendix B, section 3 2.

DEVIATION REPORT NUMBER:

C1-01-82-66 11.

LASALLE COUNTY STATION:

Unit 1 Ill. EVENT DESCRIPTION On May 26, 1982, it was determined that the Regenerative Wastes Solution Discharge Tank,.0WM03T, discharged on May 20, 1982, had an effluent total iron concentration of 31 mg/1, as determined from a composite sample of that tank.

This exceeded the N.P.D.E.S. Permi t limi t of 1.0 mg/l for total Iron and is reportable under Appendix B, section 3 2 of the Technical Specifications.

IV.

PROBABLE CONSEQUENCES OF THE OCCURRENCE This occurrence did not affect the health and safety of the general public.

This is a common occurrence to the Regen. Wastes Soln. Disch. Tank and will undoubtedly continue until the system modifications under FCR # LOO 72 are completed.

V.

CAUSE The well water hydrogen peroxide system injection pump failed due to a collapsed suction tube.

This tube collapsed due to its thin walls.

This prevented the injection of hydrogen peroxide into the well water to aid in the removal of iron and solids when it passes through the Greensand Filters.

VI.

CORRECTIVE ACTION The sucti'on tubing was replaced with a thick walled vacuum type tygon tubing

?f any arranged to minimize the number of bends and possible kinks in the tubing.

The peroxide injection system is awaiting a permanent installation on FCR

  1. LOO 72.

Action item Record 1-82-301 has been submitted to track this FCR for completion by approximately September 1, 1982.

DEVIATION REPORT NUMBER:

01-01-82-66 11.

LASALLE COUNTY STATION:

Unit 1 lit. EVENT DESCRIPTION On 5/26/82, it was determined that the Wastewater Treatrent-Facility (WWTF) had an effluent total iron concentration of 7.0 mg/1, as determined from a composite sample obtained 5/15/82.

This condition was as a direct result of the low WWTF effluent pH reported in DVR #01-01-82-40 and continued until 5/24/82.

This exceeded the N.P.D.E.S. Permi r limit of 1.0 mg/l for total iron and is reportable under Appendix B, section 3.2 of the tech. specs.

IV.

PROBABLE CONSEQUENCES OF THE OCCURRENCE This occurrence did not affect the health and safety of the general pubyc.

This is not a frequent occurrence.

V.

CAUSE On 5/12/82, the M.U.D. acid sump, 0WM06, overflowed 3 tires during regeneration of H.U.D.

train B.

This overflow went to the M.U.D. clean sump, OWM05, and eventually to the WWTF.

This resulted in lowering the pH of the WWTF to 3.5 S.U.

The WWTF was not sampled until 5/14/82, at which -time all discharges to this outfall were sampled to determine the cause.

However, all these discharges had returned to within allowable pH limits.

The low pH of the Wastewater resolubilized the coagulated iron and solids resulting in these solids passing y

through the Media Filters into the effluent. ~

VI.

CORRECTIVE ACTION No corrective action was taken to return the WWTF to inspect operation as the plant had a full inventory of water and there was no available space to transfer the water for neutralization.

The clean sump manhole cover has been securely fastened to aid in preventing further in-leakage.

A work request, #15738, has been submitted to change the acid sump level alarm as a further aid in preventing sump overflows.

Further inves tigations have shown that the orifice plates in the backwash lines could have been lef t out during the last maintenance on the Makeup Demins.

Work requests, #15895, 15896, 15898, have been submitted to replace the orifice plates.
