Confirmation of Action - License No. 39-19693-01 This letter refers to the telephone conversation on March 16, 1982, between Mr. John Potter of this office and Mr. Bill Lichtler of your office concerning an anonymous allegation of unlicensed individuals using radioisotopes listed under the subject license held by your office.
Based on the matters discussed, it is our understanding that the following actions will be promptly initiated:
1.
The well logging isotopes ( Americium-241 and Cesium-137) will be locked in their storage containers and removed from service until a named user is added by amendment to Condition 12 of your license, or until you can trans-fer possession to another NRC or Agreement State License.
2.
An evaluation of the allegations concerning a second unreported lost well logging source, failure to perform timely leak tests, and source handling errors will be conducted by your staff.
3.
A written report will be submitted to this office on the items identified above, by April 16, 1982.
Please note that the reporting requirements above are not subject to Office of Management and Budget clearance as required by P.L.96-511 If your understanding of our discussions is different from that stated above, please inform this office promptly.
