This refers to the routine safety inspection conducted by Mr. S. R. Lasuk of this office on March 10 and 29, 1982, of activities authorized by NRC Byproduct Material License No. 34-00341-06 and to the discussion of our findings with you, Dr. Donald Ruegsegger, and Mr. Ray Kreitzer at the con-clusion of the inspection. This also refers to the telephone discussion between Messrs. D. J. Sreniawski, S. R. Lasuk, and Dr. B. S. Mallett of this office and you on March 29, 1982, during which additional information was obtained showing that an unauthorized route of administration of licensed material took place in October 1981.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
In addition to the above areas, the inspector examined actions described in your letter dated June 23, 1978, regarding apparent items of noncompliance found during our May 5, 1978, inspection. We have no further questions regarding these matters.
During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in the enclosed Appendix.
A written response is required. You will note that no reply is required regarding Item 4 in the Appendix.
