Presray Letter 11/10/81 Gentlemen:
As proposed in our letter of November 10, 1981, we have now reviewed, with our customer W.J.Woolley, a list of all plants to whom seals of the type in question have been supplied.
Only those seals less then five years old are under investigation, as all plants have been advised that all seals older than five years are no longer to be considered serviceable.
We have fur-ther confined our study to seals that have a potential for being installed at operating plants, i.e.
seals shipped from Presray five years or less prior to actual or scheduled plant start-up.
In this category are seals at Midland, McGuire, Grand Gulf and River Bend.
We have instructed W.J. Woolley Company to advise the above plants that some of their airlock door seals may present a potential safety problem, and not to use any of these seals after s tart-up without a recortification by Presray.
The seals will be recertified after adequate test and inspection to verify that the problem does not exist.
If an approved test and inspection pro-cedure cannot be developed, the original seals will be replaced with the new design which has been qualified by cycle testing, installed in the configuration of use, followed by destructive examination.
