Appendix NOTICE OF VIOLATION Good Samaritan Hospital License No. 34-01311-01 As a result of the inspection conducted on March 24, 1982, and in accordance with the NRC Enforcement Policy, 45 FR 9987 (March 9, 1982), the following violation was identified:

License Condition No. 17 states that you shall possess and use licensed material in accordance with statements, representations, and procedures listed in application dated August 22, 1979 and referenced letters.

Attachment I of your August 22, 1979, application states that area survey procedures will include:

(a) daily GM surveys of dose preparation areas, and (b) bi-weekly wipe tests for removable contamination.

Contrary to the above, (a) daily GM surveys were not performed between April 22 and May 7, 1981; May 7 and 26, 1981; July 2 and August 14, 1981; August 20 and September 9, 1981; nor during several other periods, and (b) bi-weekly wipes were not performed between May 8 and September 11, 1981.

This is a Severity Level V violation (Supplement VII).

This is a repeat item of noncompliance.

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:

(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Consideration may be given to extending your response time for good cause shown.
