Damaced Low Pressure Turbine At approximately 6.13 a.m. on January 31, 1981 blading from Number 31 Low Pressure Turbine (LPT) separated from the shaft during operation apparently causing tube leakage in the main condenser.

The secuence of events is as follows:' at 6.15 a.m.

a high vibration alarm occurred on No. 4 Main Turbine Shaft Bearing which is on the south end of No. 31 LPT.

Immediately following the alarm, chloride alarms on all condensers peaked, and the operator had difficulty maintaining condenser vacuun.

Load reduction was initiated and NRC was notified via the ENS at 6 50 a.m.

The condensers were isolated to identify the chloride source.

At 8.15 a.m. a mode reduction to Hot Shut i

Down was initiated.

At 8 39 a.m. a reactor trip occurred due to high steam generator (S/G) water level on No. 31 S/G.

j Reactor power was at 9% with one main boiler feed pump in l

manual control.

Separation of turbine blading from the "last stage of No. 31 LPT was verified at about 3 30 p.m. by visual inspection.

The unit is presently in cold shutdown.
