l APPENDIX A NOTICE OF VIOLATION Basin Surveys Inc.
License No. 47-09453-01 Based on the inspection conducted on April 2 and 20, 1981, it appears that cer-tain of your activities were in noncompliance with.NRC requirements, as noted below.
10 CFR 20.101(a) requires that no licensee shall possess, use or transfer i
licensed materials in such a manner as to cause any individual in a restricted area to receive in any period of one calendar quarter, a total occupational dose to the whole body in excess of 1.250 rems.
10 CFR 20.101(b) states that a licensee may permit an individual in a restricted area to receive a total occupational dose up to 3 rems provided that the licensee has determined the individual's accumulated occupational dose to the whole body on Form NRC-4 or equ.ivalent.
Contrary to the above, the requirement to limit occupational doses to less i
than 1.250 rems in a calendar quarter was not met in that three individuals in your employ received occupational doses of 2.0,1.90 and 1.60 rems for the period January 1981, May 1980 and January 1980, respectively.
These

exposures occurred without prior determination of accumulated occupational doses on a Form NRC-4 or equivalent.
This is an Infraction (Civil Penalty -50) i Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged viola-4 tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good 1
cause shown.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
