I Appendix NOTICE OF VIOLATION St.-Joseph Mercy Hospital License No. 14-14700-01 i
As a result of the inspection conducted on May 20, 1982, and in accordance
.with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violations were identified:

License Condition No. 17 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated May 4, 1977.
Item 11 of the application
-states that the dose calibrator will be checked for accuracy at installation and annually.
i Contrary to this requirement, it was learned through statements _of licensee representatives and the NRC inspector's review of records that this requirement is not being met Specifically, the dose calibrator was last calibrated for accuracy January 24, 1981.
This is a Severity Level V violation (Supplement VI).

10 CFR 35.14(e)(1)(1) requires that sealed sources used as calibration or reference' sources be tested for leakage and/or contamination at intervals not to exceed six months.
l Contrary to this requirement, between June 21, 1979 and May 20, 1982, a 210 microcurie' cesium-137 scaled source v s not leak tested.
