This letter constitutes an interim report to the subject condition initially reported as a potentially reportable deficiency under 10CFR50.55(e).
Our previous report, QA-1699, dated April 14, 1982, stated that welders were qualified to the test requirements of H. J. Kaiser Co., procedure SPPM 3.1.51.
Contrary to the requirements of the ASME Section IX, 1971 thru 1973 Winter Addenda, welders were qualified only in two positions, vertical (3G) and overhead (4G). The Code states that qualification in the horizontal (2G),
vertical, or overhead position shall qualify-also for the flat position.
Qualification in the horizontal fixed position (5G) shall qualify for the flat, vertical and overhead positions. Qualification in the horizontal, vertical and overhead positions shall qualify for all postions. Qualification in the inclined fixed position (6G) shall qualify for all positions. Thus, several welders were not qualified to weld in the positions required to make several welds on the sacrificial shield.
In our last report we identified an ongoing investigation that was underway to resolve the deficiency and determine a resolution. The intent of the investigation was to determine if the welders could be qualified on production velds. As a result, another potentially reportable Item, M-49, was discovered and identified that a random review of radiographs, in the area where these welders performed, had identified discrepancies in the interpretation and records G206040125 820520 b
