Y Appendix NOTICE OF VIOLATION Hutchinson Area Vocational License No. 22-15554-01 Technical Institute As a result of the inspection conducted on May 13, 1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violation was identified:
License Condition No. 18 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated September 30, 1980, and letters dated October 3, 1980, May 1, 1981, August 7, 1981 and September 15, 1981.
The Operating and Emergency Procedures Manual that accompanied that-application states in Section XIII Part 2.1.1.1 that the radiation safety officer or a qualified radiographer designated by the radiation safety officer shall audit each radiographer at least once every three (3) months.
Contrary to the above referenced requirement, quarterly jobsite audits were not conducted from September 8, 1981 to March 13, 1982, a period greater than three months.
This is a Severity Level V violation (Supplement VI).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office w! thin thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
