APPENDIX A ITT Grinnell Industrial Piping, Incorporated Docket No. 99900019/82-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on March 16-18, 1982, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

Criterion V of Appendix B to 10 CFR Part 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accom-plished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantita-tive or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Nonconformances with these requirements are as follows:

A.

Section V of the ASME Code, Article 2, paragraph T-161 states in part,

" Radiography shall be performed with a technique of sufficient sensi-tivity to display the penetrameter image and the specified hole, which are essential indications of the image quality of the radiogrcphs."

Contrary to the above, radiography of pipe welds with wall thickness of less than inch was not performed with a technique of sufficient sensitivity to display the penetrameter image and the specified hole for certain piping assemblies furnished to TVA, Hartsville Plants A and B, and Consumer Power Company, Midland, Unit 1.

During a review of radiographs at the Midland site and at ITT Grinnell, Kernersville, North Carolina, it was identified that certain radiographs had been j

mechanically altered to include an artificial representation of the specified 4-T hole of the penetrameter.

Section III of the ASME Code Subsection 'NA, Article NA-4730 states in part, "The identification of conditions adverse to quality, the cause of the condition, and the corrective action taken shall be documented.

The requirements shall also extend to the performance of the sub-contractor's corrective action measures."

. Contrary to the above, documentation was not made available to the NRC inspector which would indicate that corrective action requirements had been extended to a subcontractor, who had examined fittings by the ultrasonic method instead of the required liquid penetrant or magnetic particle method.
