APPENDIX g-NOTICE OF VIOLATION Accu-Test, Inc.
Docket: 30-17040 License:
43-19144-01 As a result of the inspection conducted on April 14, 1982, and in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C),
47 FR 9987 (March 9,1982), the following violations were identified:
1.
License Condition 12 requires, in part, that licensed material be used by or under the direct supervision of certain specified individuals.
Contrary to this requirement, licensed material was used by or under the supervision of individuals othei than those specified in the license during the period October 1981 to April 1982.
This is a Severity Level IV violation (Supplement VI.D).
2.
License Condition 13(A)(1) requires, in part, that sealed sources shall be tested for leakage or contamination at intervals not to exceed 6 morths.
Contrary to this requirement, all sealed sources were leak tested but not within a 6 month interval during the period of January 11, 1980, and October 21, 1981.
This is a Severity Level IV violation (Supplement VI.D).

License Condition 17 requires, in part, that the licensee shall possess and use licensed material in accordance with statements, representations, and procedures contained in the application dated September 5, 1979, as amended by letters dated October 25, 1979, November 1, 1979, and July 29, 1980.
(a)
Item 5 of the application identifies a specific individual as the Radiation Protection Officer.
Contrary to this requirement, an individual other than the one specified had assumed the position of Radiation Protection Officer since October 1981.

(b)
Item 11 of the application specifies that radiation detection instruments were not calibrated at 6 month intervals since December 1980.
Contrary to this requirements, portable radiation detection instruments shall be calibrated every 6 months.
This is a Severity Level IV violation (Supplement VI.D).
4.
10 CFR 20.401(a) requires, in part, that records of the radiation exposures of all individuals for whom personnel monitoring is required be maintained.
Contrary to this requirement, records were not maintained showing the radiation exposures of all individuals for whom personnel monitoring was required for the period February 25, 1980 to February 12, 1981.
This is a Severity Level V violation (Supplement IV.E).
Pursuant to the provisions of 10 CFR 2.201, Accu-Test. Inc., is hereby required to submit to this office, within 30 days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
