MNl 2 41032 Holzer Medical Center License No 34-06002-01 ATIN:
Mr. Charles Adkins Vice President 385 Jackson Pike Gallipolis, OH 45631 Gentlemen:
This refers to the routine safety inspection conducted by Ms. E. Matson of this office on April 19, 1982, of activities authorized by NRC Byproduct Material License No. 34-06002-01 and to the discussion of our findings with Mr. L. Bittinger and other members of your staff at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license.
The inspection consisted of a selective examination of procedures and repre-sentative records, observations, independent measurements, and interviews with personnel.
In addition to the above areas, we are concerned about the level of radiation measured by our inspector in the " hot laboratory." The inspector measured 34 milliroentgens per hour at head level, two feet from the technetium-99m active waste storage area (a restricted area).
Therefore, in addition to your reply to the items of noncompliance enclosed, please describe what action you have taken to reduce this level and assure that radiation levels remain as low as is reasonably achievable.
During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in the enclosed Appendix.
A written response is required.
