Appendix NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-295 Docket No. 50-304 As a result of the inspection conducted on January 15 and 19,1982, and in accordance with the Interim Enforcem<
.t Policy, 45 FR 66754 (October 7, 1980), the following violation was ideni2fied:
10 CFR 50.59 states, in part: The licensee shall maintain records of changes in the facility and of changes in procedures made pursuant to this section, to the extent that such changes constitute changes in the facility as described in the safety analysis report or constitute changes to procedures as described in the safety analysis report. These records shall include a written safety evaluation which provides the bases for determination that the proposed change does not involve an unreviewed safety question.
10 CFR 50, Appendix B, Criterion V states, in part:
Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accom-plished in accordance with these instructions, procedures, or drawings.
Contrary to the above: The licensee changed the low suction pressure trip switch setpoints for the IC, 2A, 2B and 2C auxiliary feedwater pumps without following prescribed procedures (Zion Administrative Procedure ZAP 3-52-1) and without performing required written safety evaluations. As a result of this violation, on November 26,1981, the 2B an ! 2C auxiliary feedwater pumps were discovered to trip on low suction pressure following a manual start with discharge flow throttled to 100% of design flow. The ability to pass surveillance (operability) tests was unimpaired.
Remedial action was taken within Technical Specification Action Statement time limits.
