Thank you for your letter of January 22, 1982, in response to our letter dated December 23, 1981. As a result of our review, we find that the following additional information is needed:

General - Your response suggests that your review may have been restricted to the specific examples cited in the identified noncon-formances. Accordiogly, we are concerned as to whether similar deficien-cies may exist in other equipment and/or records.

It is, therefore, requested that you clarify your response, as indicated below, in order that we may note fully assess the generic aspects of the identified nonconformances and their current corrective action status.

Item A, Notice of Honconformance - Your response fails to provide any specific corrective actions taken with your staff for the identified failure to either witness established mandatory hold points, or issue j

written waivers. Similarly, your comitted action to prevent recurrence does not clearly address the stated nonconformance.

Item B, Hotice of Nonconformance - Your response partially addresses corrective actions for the cited examples, but, we believe, does not discuss the central issues, namely, the failure of procedures to assure accumulation of required records, and the failure of final record review to assure that all racords noted on the RCL were present.

Combustion Engineering, Inc. Additionally, in regard to items B.1 and B.3, you have not indicated whether the RCLs have now been corrected. We also note, in regard to item B.4, that you state welding procedures and their qualifications were reviewed by Combustion Engineering, Inc. (C-E) as indicated on surveillance reports. Our review of the applicable surveillance report (No.1, dated September 19,1978) shewed that the report did not specify which proce-dures and qualifications were reviewed.

In this regard, the example stated in item D.2 identified an instance where C-E did not review a welding procedure and its qualification.

As referenced above (General), we are concerned about the generic aspects of this nonconformance.

It is, therefore, requested that you clarify your response with respect to the extent of your review and the basis for the apparent conclusion that the problem is restricted to one specific GQC representative and contract.

Item D, Notice of Nonconfomance - Your coment concerning Component Cooling Water Heat Exchanger foming qualifications is provisionally accepted, pending our verification of the applicability of these ASME Code Section III requirements with respect to a one inch thickness shell course in these vessels. With respect to Shutdown Heat Exchangers, it is requested that you furnish us with a copy of forming qualification data when this becomes available to C-E.

Also, please identify whether your corrective actions have included verification that applicable foming qualifications, or pemissible alternate formed product testing, have been accomplished by other C-E vendors of delivered components.

.In regard to your coments on SFA 5.20 E70T welding materials, your response does not consider the applicable ASME Code,Section III impact testing requirements (NC-2310, Summer 1972 Addendum). Accordingly, you are requested to provide tne corrective action measures originally requested in our letter of,Deumber 23, 1981.
