From our telephone discussion with you on h y 6, 1982, we understand that Bendix wishes to terminate its NRC license within the next several months. Therefore, you should 1.
Complete, sign, and date.the enclosed Form NRC-314. " Certificate of Disposition of hterial." Describe what acterial was involved and when the material was disposed of.or transferred.
2.
If any unsealed radioactive materials have been used in your program, conduct a contamination survey of the facilities.
3.
Send a covar letter requesting license termination, the Form NRC-314, and survey, if appropriate, to:
